Nuts and Bolts of IM4Q History of IM4Q Planning Advisory Committee for ODP in 1997 developed Multi-Year Plan. The aim of the Plan was to determine how people being served and their families feel about the services they receive. The IM4Q is a data resource to the Administrative Entity and Providers for the Quality Improvement Plan. ODP is using IM4Q data to respond to CMS Intent and Purpose of IM4Q To Collect independent data from people who receive services from ODP and their family To ensure quality of life and satisfaction with services utilizing the Considerations Closing the Loop Process To provide data to stakeholders of the ODP system

Whats Important from SC Perspective? SCs are responsible to complete a designated amount of presurveys distributed yearly by the Administrative Entity. These pre-surveys should be completed thoroughly to ensure that all information is correct. Check HCSIS alerts daily! You can bet once youve completed your IM4Q pre-surveys that the IM4Q teams will begin to schedule and facilitate meetings with the individuals you serve. Checking alerts is important. It is best practice to enter responses within 30 days of the local program entering it. Nuts and Bolts Whats Important from SC Perspective? Respond to IM4Q considerations in HCSIS adequately and promptly. Ensure the loop is closed. Theres no loose ends that must be worked on.

Nuts and Bolts Finding Your Considerations in HCSIS Use your HCSIS credentials to log in to HCSIS. Once youre logged in you can follow the menu path to find your considerations. M4Q IM4Q CONSIDERATIONS Once you reach the considerations screen, type in your consumers last and first name, then you will see all of his/her information. Click on your consumers name, and then you will see a list of all of his/her IM4Q considerations. Click on the hyperlink under consideration ID that will take you into the considerations box. Click the select bubble next to the consideration, and then click edit to begin working on his/her IM4Q in the box that says AE/SCO response. Nuts and Bolts

How to Write a Thorough HCSIS Response A great and thorough response has these characteristics.. Has excellent details. Always be as specific as possible. Include dates, names, the entire plan to resolve the issue etc. The response shows a willingness by the SC to follow-up and close the loop The response is readable. Double check your writing. Check for proper grammar and spelling errors. Consideration Theme: Request for information Consideration #1 Consideration: Matties mother mentioned in the family survey she would like information on guardianship. Response: THE SC CALLED MATTIE'S MOTHER ON 7/10/2019 TO DISCUSS GUARDIANSHIP FOR MATTIE AND DISCUSSED THE NECESSARY STEPS NEEDED TO ESTABLISH GUARDIANSHIP. THE SC ASKED MATTIE'S MOTHER IF SHE WOULD LIKE THE INFORMATION SHE RECEIVED LAST YEAR ABOUT GUARDIANSHIP AGAIN AND SHE REPLIED THAT SHE WOULD. ON 7/10/2019,THE SC LOCATED THE INFORMATION AGAIN AND MAILED TO HER FOR HER REVIEW.ALSO ON THIS DATE, THE SC LOCATED AND REVIEWED ADDITIONAL DOCUMENTATION RELATED TO GUARDIANSHIP AND ALTERNATIVE OPTIONS. THE SC RESEARCHED THE PROCESS CONNECTED TO ESTABLISHING GUARDIANSHIP AS WELL AS REVIEWING DOCUMENTATION FOR ESTABLISHING AN ADVANCED HEALTH CARE DIRECTIVE, FINANCIAL GUARDIANSHIP, REPRESENTATIVE PAYEE, AND THE LIMITATIONS OF GUARDIANSHIP/DIRECTIVES. THE SC FOLLOWED UP WITH MATTIE'S MOTHER DURING A MEETING FACILITATED ON 7/15/2019, TO ANSWER HER QUESTIONS AND REVIEW THE STEPS NECESSARY TO ESTABLISH GUARDIANSHIP AS WELL AS PROVIDING HER MORE INFORMATION ABOUT ALTERNATIVE METHODS TO SUPPORT MATTIE THAT ARE LESS RESTRICTIVE. Always complete your IM4Q consideration responses honestly and in its entirety. Check your HCSIS alerts often to check if you have IM4Q considerations that must be addressed. Be timely in writing your responses (within 30 days) Always make sure that the individuals we are serving are satisfied with the outcome. Its their life, let them live it well!
