GENERAL INSTRUCTIONS1An original of this report form properly filled out and verified shall be filed with theSecretary of the New Jersey Board of Public Utilities, 44 South Clinton Avenue, 9th Floor,Post Office Box 350, Trenton, New Jersey 08625 on or before the last day of the thirdmonth following the close of the calendar year.One copy of the report should be retained by the respondent in its files. NOTE: If thefollowing schedules: VIZ., Important Changes During the Year, Comparative BalanceSheet, Notes to Financial Statements, Summary of Utility Plant and AccumulatedProvisions for Depreciation, Amortization and Depletion, Statement of Income for theYear, Statement of Retained Earnings, and Electric/Gas Operating Revenues are filedon or before the foregoing established filing date, the filing of the complete report, whichshall include the above expected schedules originally filed, should not be postponedbeyond the above established filing date without the formal request in writing in a timelymanner and, authorization from the Board.2This form of annual report is prepared in conformity with the Uniform System of Accountsfor Electric/Gas Utilities prescribed by Federal Energy Regulatory Commission andadopted by the New Jersey Board of Public Utilities, and all accounting words andphrases are to be interpreted in accordance with the said classifications.3Instructions should be carefully observed and each question should be answered fullyand accurately, whether it has been answered in a previous annual report or not. Wherethe word ‘none” truly and completely states the fact, it should be given in response to anyparticular inquiry. Where dates are called for, the month and day should be stated aswell as the year. Customary abbreviations may be used in stating dates.4If any schedule does not apply to the respondent, such fact should be shown on theschedule by the words ‘not applicable’. A notation indicating same should also be madein the “Remarks” column on the list of schedules. All schedules deemed to be notapplicable should be included in the annual report filing.5The spaces provided in this report are designed to be filled in on a typewriter havingelite-size type, and such a typewriter should be used if practicable.6Reports should be completed by means which result in a permanent record. The originalcopy shall be made out in permanent black ink or with permanent black typewriterribbon. Entries of a contrary or opposite character (such as decreases reported in acolumn providing for both increases and decreases) should be enclosed in parentheses.7Commission authorization (abbreviated, Comm. Auth.) used in the report means theauthorization of the New Jersey Board of Public Utilities or any other regulatory body.Where a commission authorization is shown, the identity of the commission should alsobe given.i

GENERAL INSTRUCTIONS (Continued)8.This form should be filled out so as to provide an annual report complete in itself.References to report of previous years or to other reports, except as herein otherwisespecifically directed or authorized, should not be made in lieu of required entries.9.Figures of a previous year reported for comparative purposes shall agree with thoseshown in the annual report of that previous year, or be accompanied by an explanationof the reason why they do not agree.10Additional statements inserted for the purpose of further explanation of accounts orschedules should be made on durable paper conforming to this form in size and width ofmargin. The inserts should be securely bound in the report. Inserts should bear the titlesof the schedules and report form page numbers to which they pertain.11If respondent so desires, cents may be omitted in the balance sheet, income statements,and supporting schedules. All supporting schedules on an even-dollar basis, however,shall agree with even-dollar amounts in the main schedules. Averages and extractedfigures, where cents are important, must show cents for reasons which are apparent.12The “Date of Report” on the top of each page is the Annual Report due date.13The word “Respondent’ wherever used in this report, means the person, corporation,agency, authority, or other legal entity or instrumentality on whose behalf the report ismade.14In addition to filing this form, the respondent shall also file with the Board, immediatelyupon publication, four copies of its latest annual report prepared for distribution (byrespondent or its parent) to stockholders, bondholders, or other security holders.
